Makenzie would like to return to the children’s library for another one day job shadow on 4/9 from 10:30-2:30. She would also like to request the internship placement this summer. She is eligible for the funding my program offers for up to 30 hrs per week with your organization. This request pertains to the young lady who job shadowed in nursing on 12/15. She would like to also experience her internship at LH Rehab and Senior Living over the summer in patient care. In years past, those interested in health care have worked in the rehab center. The dates provided on this form are estimated but indeed will be 8 weeks in length.
