The below Career Experience Release Form must be signed by the parent or legal guardian of each student which gives consent to allow the school to provide a username and password to the student providing them with full access to the website including buttons that allow them to request career experiences and participate in meaningful career exploration activities. This release is a template that can be customized by your school. We encourage you to include your school's letterhead.
